Job Description
Assists with coordinating patient Palliative Care screening and performing visits when appropriate. Identifies as soon as possible any spiritual care needs for Palliative Care (PC) patients, and minister to each identified patient’s spiritual and emotional needs as warranted by the initial and ongoing spiritual assessment.
Implement and support an interdisciplinary approach to pastoral care
Provides pastoral intervention and care during times of crisis for patients and families, and makes referrals, as deemed necessary, to other members of the Palliative Care Team
Attends daily Team Meetings on working days and communicates with staff regarding fulfillment of care planning for each patient.
Accurately coordinates the volunteer chaplain schedule. Maintains a contact list of clergy of various faiths and acts as a liaison with other team members for religious needs, issues, or questions.
Function as coordinator of scheduling, supply ordering, and lead meetings and training for other chaplain volunteers. Actively participate in the delivery of pastoral care during working days. Ensure that all requests for pastoral care are responded to in a timely manner. Ensure that ritual and sacramental needs are met.
Remains flexible with work schedule; accepts and participates actively in change within the department and hospital.
Represents UM CRMC in a positive way within the Health System and community.
Qualifications
Bachelor’s degree or equivalent is required in Theological Education or related field. Acute care chaplaincy experience required. Experience in a health care setting is preferred.
