Job Description

Under limited supervision assesses, develops, implements and coordinates care plans for patients receiving extended medical and/or behavioral care, provides direct care to patients experiencing emotional and social problems and supportive counseling to patient family members. Collaborates with other staff concerning patient admissions and discharges from the treatment facility. Through advanced practice skills mobilizes resources to reduce risk, and serves as ambassador between hospital and community.  Provides clinical supervision as required by the Board of Social Work Examiners.

Qualifications

Principal Duties:

  1. Assess the patient’s psychosocial situation in relation to the medical and psychiatric needs by providing brief admission assessments of each patient, family, or significant other to extend support and determine willingness. Ability to understand and comply with the D/C treatment plan, and understand availability of the caregiver to care for the hospitalized patient upon discharge. 
  2. Meets with, mentors and reviews compliance of clinical documentation of lesser-licensed team members in accordance with state and federal licensure requirements.
  3. Conducts comprehensive psychosocial evaluations and documents appropriate data from psycho-social assessments, individual contacts, family interventions and discharge planning according to department policy and procedures and State, Federal, and Joint Commission stipulated requirements.
  4. Interviews patients, family members, and/or significant others identified through case finding or referral to assess needs for social work services and plans appropriate interventions and treatment goals to improve psychosocial outcomes.
  5. Provides on-going community awareness to services including distributing information to feeder agencies and at public events. Provides preventive, protective, or supportive services, including mandatory reporting for patients who are in or who are threatened by situations detrimental to their well-being.
  6. Assists in the coordination of patient discharges with healthcare providers/agencies for support in the patient's home.
  7. Identifies the need for and conducts family meetings that result in comfort, decisions, and other important outcomes
  8. Ensures continuity of care throughout the continuum of care when required by patient population
  9.  Provides family counseling as needed to help explore and resolve concerns regarding the family member residing at the Center.
  10. Completes information for monitoring quarterly and annual utilization review and other reports.
  11. Provides consultation to Social Work students and LGSW staff on difficult psychosocial cases.
  12. Develops a social service network with city, state or other governmental services that serve the needs of the assigned 
  13. patient population, to assist patients in obtaining such services as, food stamps and other financial supports and assistance in obtaining transitional housing.
  14. The LCSW-C will lead the Perinatal Bereavement Committee and is essential to provide specific skills such as grief, loss, and bereavement counseling, both for the family unit as well as the staff, and linkage to community resources.
  15. The  LCSW-C is essential to the Perinatal team providing assistance to the family focusing on specific tasks that may be time sensitive such as funeral planning, sibling and grandparent inclusion, and assisting the family in memory making
  16. Functions within the Women’s and Infants Services Division to support mothers as needed
  17. Facilitates and Precepts SW students acclimating the students to the needs of mothers in PG County
