Reports to: Nurse Manager
Hours: 37.5/week
Shifts: 8-10-12 Hour Day/Nights/Weekends/Holidays
Salary: $45.85 to $57.31 per hour
Status: Temporary Full-Time (3‑month contract with the possibility of extension)

Position Summary
In the Toronto Western 38 bed Medical Surgical Neuro ICU (MSNICU), the interdisciplinary team provides care to patients who require advanced ventilatory and hemodynamic support to overcome the complex challenges of neurosurgical emergencies, shock syndrome, and multisystem failure.
Reporting to the Unit Manager, as a member of the multi-disciplinary team on the MSNICU, the Patient Care Coordinator – Critical Care Response Team (CCRT) Co-Lead acts as a resource for the multidisciplinary team. This position plays a leadership role by mentoring and providing direction, ongoing development and preceptorship to team members.
60% of the time will be allocated to the CCRT Co-lead responsibilities and 40% to the PCC responsibilities as described below.
Duties
CCRT-Co-Lead Responsibilities and Duties (60%)
As an integral member of the Critical Care Response Team and in collaboration with the medical CCRT lead, the unit manager as well as CCRT co-leads at Toronto General Hospital, the CCRT Co-Lead overseas the daily activities & functioning of the MSNICU CCRT team and liaises with key stakeholders to ensure the delivery of high-quality patient care and optimal management of patient care outcomes. The CCRT Co-Lead plays a leadership and administrative role by:
- providing direction, ongoing development, and mentorship to team members
- ensuring 24/7 CCRT scheduling coverage
- developing criteria for the selection of CCRT providers
- marketing and promotion of CCRT to stakeholder groups
- ensuring resource availability to cover training requirements
- developing and delivering educational in-services/sessions for both CCRT providers and ward staff
- leading or participating in as well as reporting on quality improvement initiatives related to CCRT services
- liaising with ward leadership to identify learning needs
- ensuring accurate data collection and reporting with respect to CCRT activities and core team functions
- performing cross-functional and/or other duties consistent with the job classification as assigned or requested.
Patient Care Coordinator (PCC) – Responsibilities/Duties (40%)
- liaising with members of the multi-disciplinary team to ensure high standards of quality and optimal management of patient care outcomes
- contributing to nurses, students and ancillary staff development by promoting and ensuring a supportive and collaborative learning environment within the service(s)/cluster area based on the principles of adult learning and practice standards
- performing human resources, financial and administrative responsibilities to ensure efficient and effective unit operation
- performing cross-functional duties consistent with the job classification as assigned or requested

Qualifications
- Baccalaureate in Nursing required, Masters preferred/prepared
- Three years nursing experience in a tertiary care ICU
- Current MSNICU experience preferred
- CCRT experience preferred
- CCRN an asset
- Demonstrated leadership abilities required with prior leadership experience preferred
- Demonstrated ability towards team building and collaboration
- Effective organizational, interpersonal, and communication skills
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
- Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
- Exercise initiative and good judgment with ability to multi-task
- Excellent decision making and problem-solving skills
- Clinical and patient teaching skills
- Excellent time management skills
- Excellent skills in MS Word, Excel, SharePoint, EPIC, and Incident Reporting software
