Salary Range: $40.34 - $50.43 per hour
Hours: 37.5 hours per week
Shifts: Days
Status: Permanent Full-Time

As an integral member of the Cardiac Rehab team, the responsibilities of the CLINICAL INTAKE COORDINATOR,  include: 

  • Utilizing clinical knowledge and experience to serve as an entry point for the timely access to appropriate clinical assessment and treatment services in cardiac rehab.
    • Reviewing patient cases to decide if they are clinically ready for rehab. 
    • Working along with the Service Coordinator to Triage the receipt of approximately 75 referrals per week. 
  • Obtaining and maintaining current knowledge of internal/external services to communicate timely information.
    • Re-directing patients to appropriate care, or following until ready for rehab. 
  • Collaborating with representatives from all services to plan, develop, implement and evaluate a systems approach to client care.
    • Participates as a member of the interdisciplinary team; demonstrates leadership and serves as a role model for cooperation and client focused problem solving. 
  • Identifying gaps in service and participating in addressing such, internally and externally.
    • Continuously monitoring model of care and providing solutions to create an efficient system. 
  • Playing a collaborative/facilitative role with respect to the management of resource utilization.
    • Continuously monitoring caseload, assignment of patients within the program. 
  • Perform cross-functional and/or other duties consistent with the job-classification, as assigned or requested.
    • Filling in to cover clinical care on occasion.
    • Participating on committees and/or working groups. 

Qualifications

  • At minimum, completion of a Bachelor’s degree in Health Sciences or recognized equivalent required. Master’s Degree preferred 
  • At minimum 4 years practical and related experience 
  • Registration/licensure as required by relevant professional body, e.g. College of Kinesiologists of Ontario, College of Physiotherapist of Ontario, College of Nurses of Ontario etc. 
  • Must have experience in direct clinical care, including intake functions/triaging as well as in community relations and service development 
  • Excellent knowledge of cardiovascular disease prevention, assessment & rehabilitation required 
  • Proven ability to work with target population 
  • Proven ability to make quick assessment 
  • Proven ability to work independently and with self-direction 
  • Proven skills in problem recognition and problem solving 
  • Proven ability to work with the interdisciplinary team 
  • Client service-oriented with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds, and characteristics bring richness to the challenge or situation at hand 
  • Exercise initiative and good judgment with ability to multi-task 
  • Compliance with confidentiality requirements 
  • Effective organizational, interpersonal, and communication skills 
  • Committed to role modelling respect, civility and inclusion in the workplace 
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards.
