Reports to: Manager, Outpatient Allied Health and Clinics
Salary: $46.85 - $58.56 per hour
Hours: 37.5 hour per week
Status: Temporary Full-Time (1 Year)

The Service Coordinator is a program specific leadership role supporting the outpatient ABI and Multiple Sclerosis programs in various aspects of the unit/program function. In accordance with established standards, professional requirements and as an integral member of the multidisciplinary health care team(s) the SERVICE COORDINATOR is responsible for the overall patient flow of the program inclusive of planning, organizing, directing, coordinating and evaluating activities related to patient admission and discharge. In addition, this role provides administrative and clinical leadership to all staff in a supportive, empowering manner while facilitating the effective and efficient admission and discharge of patients.
Key responsibilities of this position include:
- Leads and/or participates in developing and implementing organizational initiatives regarding quality improvements and program development
- Plans, organizes, directs, coordinates, and evaluates activities related to patient admission and discharge
- Ensures efficient and effective unit/program operation by providing mentorship and promoting a supportive learning environment for health professions, physicians, nurses, support and administrative staff
- Acts as a clinical resource for the acute and/or community inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs
- Establishes and maintains a systematic review and assessment of department capacity.
- Service Coordinators consult with and engage appropriate staff members, physicians and others and they independently make decisions for the patient’s admissions/discharges and team allocation
- Service Coordinators develop and maintain fundamental strong relationships with patients/families, external partners and other stakeholders by consulting, problem solving cases and providing ongoing education
- When a Program Manager is unavailable, the Service Coordinator of a program may act as Manager’s delegate on clinical and practice issues of the program
Qualifications
Education:
- At minimum, completion of a Master’s degree in Health-related discipline or recognized equivalent required
- A Master’s degree in a Health-related discipline is preferred
- Current valid Certificate of Practice from relevant Health related College (i.e., OT, PT, RN, SLP, SW, RT, RD etc.), required
Experience:
- At minimum, over 4 years up to and including 6 years practical and related experience and/or 2 years on-the-job training required
- Knowledge and experience in admission and discharge planning and case management required
- Knowledge and experience using utilization management principles and length of stay management strategies required
- Demonstrated leadership abilities and experience required
- Experience in project management preferred
- Experience in program development and implementation preferred
- Experience working with interdisciplinary teams is required
- Specialized experience with the rehabilitation ABI/MS population the role is with is required
- Excellent decision making, organizational and time management skills required
- Knowledge of standards of practice and applicable healthcare legislation required
- Current Basic Life Support (C Level) Heart Saver plus (Canada) required
Professional Affiliations/Memberships:
- Membership in relevant Ontario College of Health-related discipline required
- Membership in relevant professional agencies and associations preferred
