Position Summary
The Al & Malka Green Artists' Health Centre offers integrated care to artists of all disciplines, and to arts students and educators. The Centre is the only clinic of its kind in Canada that specializes in serving artists. With a mandate that also includes research, education and engagement in the arts community, the Centre strives to understand the health needs of artists to provide evidence-based, artist-centered care.
The Clinical Intake Coordinator uses clinical knowledge and experience to serve as an entry point for the timely access to treatment services offered at the AHC.
Duties
As the first point of contact for new patients, the Clinical Intake Coordinator:
- Receives and reviews the applications for all patients interested in joining the AHC
- Conducts an intake appointment which includes a review of application, health assessment, recommendations for services and coordinates follow-up
- Explains the model of care and services offered at the Artists’ Health Centre
- Communicates with other members of the team
Qualifications
- At minimum, completion of a Bachelors degree in Health Sciences, or recognized equivalent
- Exercise initiative and good judgment with ability to multi-task
- Effective organizational, interpersonal, and communication skills
- Knowledge of applicable legislative, UHN and/or departmental policies
- Client service-oriented with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds, and characteristics bring richness to the challenge or situation at hand
- Proven ability to work independently and with self-direction
- Proven ability to work within interdisciplinary teams
- Proven ability to work with artists and artistic communities

Current UHN employees must have successfully completed their probationary period, have a good employee record along with satisfactory attendance in accordance with UHN's attendance management program, to be eligible for consideration.
