Position Summary
The Antimicrobial Stewardship Program at UHN is seeking an Antimicrobial Stewardship Pharmacotherapy Specialist to join the team and be a key player in establishing initiatives that foster safe and optimal antimicrobial use, lower antimicrobial resistance, increase patient safety and cost-efficiency across all sites of the University Health Network. The successful candidate acts as an antimicrobial stewardship resource for internal and external health care professionals in a patient-focused interprofessional practice.
The Antimicrobial Stewardship Pharmacotherapy Specialist contributes to the establishment of clinical practice guidelines that ensure appropriate antimicrobial therapy. This includes: antibiotic prophylaxis, establishing guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy associated with a definite diagnosis, optimizing antimicrobial dose and route of administration based on pharmacokinetics and pharmacodynamic principles.
The successful candidate will collaborate with leaders and clinicians in Antimicrobial Stewardship, Infectious Diseases, Clinical Programs, Microbiology, Infection Prevention & Control, Quality & Safety, Digital and the Pharmacy Department to provide implementation strategies for various initiatives. They may include but not limited to providing staff education, prospective and retrospective evaluation of antimicrobial prescribing practices with feedback to prescribers and clinicians regarding appropriate use of antimicrobial agents.
Duties
- Clinical Practice
- Acts as an antimicrobial stewardship resource for internal and external health care professionals in a patient-focused practice
- Works effectively and collaboratively with the patient care inter-professional teams
- Provide clinical direction on complex cases related to their clinical practice
- Liaises with the Infectious Disease Consult service and provides teaching on antimicrobial pharmacotherapy and antimicrobial stewardship to the Infectious Disease Fellows, Residents and Medical Students
- Participate in Infectious Disease Service patient care rounds on a rotational basis
- Establishes initiatives that foster improved antimicrobial use, lower antimicrobial resistance, increase patient safety and promote cost savings.
- Lead Antimicrobial Stewardship patient care rounds and provides patient specific feedback to the clinical team (audit and feedback).
- Participates in the development, implementation and evaluation of evidence-based disease specific best practice empiric treatment algorithm guidelines incorporating UHN pathogens and resistance patterns.
- Provides teaching in the healthcare professional curriculum (medicine, pharmacy, nursing, others).
- Identify opportunities for scholarly activities within antimicrobial stewardship and infectious diseases management.
Qualifications
- Completion of a Bachelor of Science Degree in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD) from an accredited program of study or recognized equivalent required.
- Certificate of Qualification with the Pharmacy Examining Board of Canada.
- Registration with the Ontario College of Pharmacists, Part A.
- Completion of a PharmD degree preferred.
- Minimum three (3) years of hospital practice, which may include residency year(s) preferred.
- Completion of an accredited Year 1 Hospital Pharmacy Residency Program (PGY1) preferred.
- Certificate from the Making a Difference in Infectious Diseases (MAD-ID) Program preferred.
- Completion of a post-graduate Pharmacy degree (e.g. Master’s degree) considered an asset.
- Completion of a Year 2 Pharmacy Residency Program (PGY2) or fellowship with specialization in Antimicrobial Stewardship and/or Infectious Diseases considered an asset.
- Has the ability to work on complex problems with various clinical and technical stakeholders to provide innovative and creative solutions to achieve resolution and maintain patient safety considered an asset.
- Knowledge of quality improvement, implementation science, research methods considered an asset.
- Demonstrates leadership, teaching, and collaborative team skills considered an asset.
