Position Summary

The Lead, Knowledge Mobilization is responsible for the development and implementation of a network-wide knowledge mobilization (KM) strategy to ensure that UHN Connected Care products are relevant and applicable for knowledge users (e.g., decision-makers, clinicians, industry partners, etc.). Reporting to the Manager, Strategy & Partnerships, the Lead will promote the scale and spread of Connected Care Program; leveraging the expertise of a broad partner network and applying knowledge mobilization best practices to enhance UHN Connected Care activities, working closely with internally and externally-based partners.

Key Roles and Responsibilities:

Knowledge Exchange, Capacity Building & Partnerships

  • Work with the Connected Care team to deliver training initiatives that build implementation capacity—including communities of practice, workshops, and webinars
  • Apply adult learning principles, develop training content, and use facilitation techniques to engage a range of audiences
  • Maintain a centralized repository of practical resources, tools, and lessons learned

Product Development

  • Oversee the redevelopment of Connected Care's website, working with a web development firm to shape content and structure that reflect its role as key partner in delivering change at scale across the health system
  • Develop engaging, accessible materials (e.g., guides, presentations, brochures) tailored to diverse stakeholders such as government, hospital partners, community agencies, and international audiences

Communications

  • Develop audience-specific messaging across stakeholder groups, ensuring clarity, consistency, and relevance
  • Support engagement through social media, website, and newsletter content
  • Create written and visual content that highlights Connected Care’s work and impact in communities
  • Identify and help coordinate speaking opportunities for Connected Care team
  • Work with UHN Communications on media requests and external engagement

Qualifications

  • Completion of a master’s degree in Communications, Knowledge Mobilization or equivalent.
  • 4 – 6 years of practical and related experience required.
  • Strong foundation in knowledge mobilization and change management frameworks, with proven ability to:
    • Develop and execute effective KMb strategies across diverse stakeholder groups including government/policy makers, health care, housing, service partners, community organizations, and the public
    • Support stakeholders through change and implementation processes
  • Experience working within or knowledge of Ontario health system and its key stakeholders

Technical Expertise:

  • Demonstrated success creating engaging knowledge products (toolkits, infographics, videos, plain language resources)
  • Experience facilitating both structured learning (workshops, training) and communities of practice
  • Strong project management skills 
  • Proficiency with digital platforms for knowledge sharing and engagement
  • Experience with evaluation and impact measurement

Professional Skills:

  • Outstanding ability to translate complex information into clear, compelling content for diverse audiences
  • Strong facilitation and presentation skills
  • Proven ability to build and maintain relationships with stakeholders at all levels
  • Experience working effectively in cross-functional, collaborative environments
