UHN Connected Care
UHN Connected Care is known for our ability to deliver sustainable change management programs. Born out of a variety of province-wide projects, we are focused on the healthcare experience as a whole. We have worked with and for stakeholders across the care continuum and garnered strong regional knowledge, including electronic referrals, electronic health records and emergency management systems.
Early years saw us introducing core foundational items to achieve transformation. While there was a heavy technology lift to our projects, our work has always been successful because of an integrated project and change management delivery approach. We continue to look to the future to see where healthcare needs to go, seeking solutions within and beyond digital health to enable what we want to do.
Today, we break down barriers to change and connect people who want to solve these problems, bringing policy makers, funders, thought leaders, clinicians and patients to the table to build solutions together. Working with our partners to bring life to great ideas, we use these ideas to create change through sustainable, shareable practices.
To inspire and invent tomorrow’s care we look to three streams of work:
1) With integrated care, we are collaborating with healthcare providers and system administrators from partner organizations to improve patient transitions from UHN into the community. Our goal is to improve patient satisfaction and their overall health outcomes by following the patient throughout their journey, including wherever possible providing needed care at home and digital solutions including remote care monitoring.
2) Our digital delivery team provides leadership and is driving change through partnerships at the local, regional and provincial levels and bring sectors together to design and implement an integrated system of care enabled by digital health. This dedicated and experienced team is able to pivot to priorities as they emerge and provide experienced delivery capacity to ensure meaningful adoption and sustainment of digital assets for Ontario Health Teams and health service providers in our health communities.
3) Our Operations team advances the eReferral footprint in the Toronto region via the Resource Matching and Referral program. In addition, this skilled team provides leadership and operational support for the Integrated Care program.
