Gattuso Centre for Social Medicine
The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity. 

Position summary

The Project Manager position will report to the Manager, Program Delivery and play a key role in leading implementation efforts aimed at optimizing and expanding the Social Medicine Care Model. This individual will be accountable for leading multiple projects by applying project and change management best practices along with comprehensive data analysis to ensure the delivery of sustained and measurable impact. The Project Manager will guide the team through the project lifecycle, providing leadership on budgeting, stakeholder engagement, coordinating with legal and procurement, and managing scope and risks proactively, while also coaching members of project team to drive successful outcomes. The Project Manager should have strong change management and facilitation skills, and navigate ambiguity with ease, and finds clarity for the team. 

Duties

  • Leads the implementation of Social Medicine projects aimed at expanding and optimizing pathways to support vulnerable patient populations.
  • Lead all aspects of project lifecycle, including scoping and planning, resource allocation, stakeholder engagement and management, monitoring and control, development of metrics and analysis, budgeting, procurement and legal processes, and post project evaluation
  • Create and facilitate review and approvals for detailed project artifacts including, but not limited to, project charters, plans, schedules, budget, evaluation framework, and operational transition plan
  • Apply rigorous project management, change management, and effective communication across the project lifecycle to manage schedule and milestones using appropriate tools, anticipating and troubleshooting issues to minimize UHN and Social Medicine’s  exposure to risk
  • Drive business insight and demonstrate benefits of implemented initiatives to the organization by applying sound data analysis, data quality, data reporting and visualization
  • Facilitate stakeholder engagement and elicit business requirements within a multi-disciplinary team environment, including leadership, clinicians, business teams, community partners, patients
  • Effectively implement and sustain change by applying rigorous change management and communication practices
  • Deliver progress reports, requirements documentation, and presentations to project stakeholders, sponsors and executive-level leaders
  • Manage procurement of goods and services, vendor, partner contracts, relationships, and delivery
  • Handle complex problems and provides highly innovative and creative solutions that will achieve quality and work process improvements
  • Facilitate development of operational model and plans for transitioning Social Medicine programs to a sustainable state
  • Work independently and within a team setting
  • Coach and guide members of the project team

Qualifications

 

  • 5+ years of project management experience or equivalent required
  • Proven track record of successfully leading projects involving multiple stakeholders, including clinical teams, vendors, and external partners.
  • Strong experience with budgeting, financial oversight, and procurement processes, including contract negotiations.
  • Strong analytical, prioritization and organization skills, with attention to detail
  • Experience with change management strategies and implementing new practices within a healthcare environment.
  • Strong leadership, coaching, and team development skills, with the ability to manage both direct and indirect reports.
  • Strong knowledge of, and familiarity with, community resources to address the social determinants of health
  • Dynamic knowledge of community engagement principles
  • Demonstrated commitment to and understanding of equity, anti-racism, and anti-oppression principles and practices
  • Bachelor's degree in Science, Health, Business Administration, or a related field. A Master’s degree is preferred.
  • Project Management Professional (PMP) certification or equivalent preferred.
  • Other certification (Lean/Six-Sigma Black Belt, PROSCI) an asset
  • Well-developed decision-making, problem-solving, and judgement skills, as well as excellent time management and organizational skills required
  • Supervisory or managerial experience preferred
