Job Description

TITLE: Guided Empowerment Model (GEM) Housing Coordinator
REPORTS TO: Care Coordination Manager
DETAILS: Full-Time Position/non-exempt (40 hours per week) 
LOCATION: Detroit Office

DESCRIPTION:

The Housing Coordinator is part of the GEM Team and he/she/they is assigned to assist clients who have treatment adherence issues, declining health and need assistance with comorbidities. The Housing Coordinator is responsible for assisting clients with securing stable housing, assuring effective use of available resources, utilizing community referrals, and maintaining established standards of care.

ESSENTIAL FUNCTIONS:

  • Meet with each client that identifies a need for housing and determine the client’s needs based on each individual situation.
  • Work with clients to determine any barriers that may prevent them from engaging and remaining in HIV primary care.
  • Analyze the tasks that need to be completed by the client, and which by the case manager.
  • Make referrals to agencies and organizations throughout the community that can provide the client assistance with achieving their goals.
  • Maintain contact with providers to ensure the client has completed the referral process.
  • Responsible for composing a housing plan with the client, and monitor follow through.
  • Evaluate and assess what difficulties clients have with securing stable housing.
  • Housing Coordinator will represent Unified on the Housing Alliance Community Task Force.
  • Assist client with emergency financial assistance as available.
  • Address financial situation and if client has income.
  • Coordinate care of assigned clients and provides referrals to any substance abuse or mental health professionals, in collaboration with the GEM Team.
  • Effectively communicate with clients on a regular basis to determine which needs have been met and to identify any new needs.
  • Meet with each client every six months to review the client's situation.
  • Revise service plan with client to reflect any changes in client situation.
  • Provide advocacy for clients as needed.
  • Conduct outreach and case finding activities as needed to keep clients connected to care.
  • Collect and submit data reports on a weekly and monthly basis for program monitoring and evaluation.
  • Input and maintain client documentation in the data system. 
  • Participate in multidisciplinary case conference meetings.

Qualifications

REQUIRED QUALIFICATIONS:

  • Bachelor’s degree in human services field or commensurate experience with HIV positive individuals.
  • Peer LWH
  • Must be able to function independently, be self-motivated, be able to work well with individuals from a variety of socioeconomic groups, and be highly organized.
  • Knowledge of community resources is helpful.
  • Must have a valid driver’s license and reliable transportation.
  • Basic computer literacy required, particularly in Microsoft Word, Excel and Google Products
