The Clinical Coordinator provides a supportive role to the clinical management team and performs a variety of duties in the coordination of the delivery of care to patients and families in their homes.  Triages calls related to patient care, including physician orders, staff issues and other clinical issues. Completes patient reviews, assesses staff competency and assists with other clinical projects as assigned. 

Essential Functions

  • Provides effective coordination of clinical services, consistent with the objectives of agency.
  • Works closely with clinical staff to provide necessary services for patients and families in a timely manner, including the provision of direct patient care.
  • Communicates regularly with Clinical Managers as well as with the Clinical Director when issues arise relative to clinical practice or other performance related issues.
  • Demonstrates expertise and knowledge in a variety of areas, including, but not limited to:
    •  Current methods of care and treatment
    • Policies, procedures and protocols which are specific to the Agency.
    • The Agency Infection Control Plan.
    • A working knowledge of all reimbursement criteria.
    • A working knowledge of all regulations.
  • Has a working knowledge of the Medicare Conditions of Participation.
  • Assists with individual caseload reviews or other audits as assigned by the manager or Clinical Director.
  • Participates in review of managerial and clinical data to support effective implementation of clinical priorities, including but not limited to HHA supervision, wound measurement and formula compliance, appropriate utilization of clinical pathways, and telehealth.
  • Interprets findings from reviews and assigned audits and makes recommendations to the manager or
  • Clinical Director.  Follows up with staff as appropriate.
  • Assists Clinical Managers and Clinical Support Coordinators to ensure appropriate coverage based on: census; skill mix; geographic distribution; skill level of available clinicians; and, patient care requirements.
  • Assists with orientation and preceptor of clinical staff.
  • Assists with orientation and supervision of the Home Health Aide services for patient’s Performs home supervisory visit
  • Works with Quality Management and the clinical managers to ensure accurate and timely paper-flow, as well as, completion of documentation in the electronic record.
  • Contributes to Performance Improvement Activities
  • Demonstrates knowledge of risk management activities; completes variance reports accurately, appropriately, and in a timely manner.
  • Contributes to the continuing evaluation of programs and performance.

Position Type/Expected Hours of Work & Travel

The hours of work are 8am to 4:30pm Monday thru Friday with some flexibility based on position.  

Qualifications

  • A current professional RN/LPN licensure in the states of Massachusetts required. Current professional RN/LPN licensure in the state of New Hampshire required for some territories.
  • Minimum of 3 years’ of community health experience required or combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience required. 
  • Prior experience in telephone triage preferred.
