• Hourly Wage: $44/hr
• Status:  This is a PRN, Per Required Need (“On-Call”) position
• Shift: We have 8-hour shifts. Our greatest need is for the PMs & Overnight shifts.
• Start Date: ASAP, we have a new employee orientation class starting soon.
  • Orientation note:  Our New Employee Orientation classes are held on Wednesday’s from 10:30-4pm.  You will be expected to attend the full session.

• Community Care Center is an inner city SNF with a diverse population of geriatrics (long and short term) as well as behavioral health.  This close-knit team is welcoming and always willing to jump in to help when you need it.

PRN (Per Required Need) / On-Call Position

This is an as-needed, on-call role with no guaranteed hours.

Minimum Availability Requirements:

• Commitment to at least two (2) weekend shifts per month (Saturday and/or Sunday)
• Intermittent weekday availability as needed
• Flexibility to work Day, PM, and NOC shifts (PMs and NOCs are most needed)
• Availability to work some holidays
• Availability to attend a Wednesday New Employee Orientation

Additional Details:

• Shifts may be scheduled in advance or offered with short notice
• No guaranteed hours
• No benefits or insurance offered for PRN status
• Non-union position

Floor Nurse:  We will consider a Registered Nurse (RN) or Licensed Practical Nurse (LPN) for this role.

What we need from you:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
• Prior floor nurse experience within a skilled nursing facility is strongly preferred
• Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex
• Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
