This Skilled Nursing Facility (SNF) is owned by Stephenson County, but managed by Transitional Care Management.  And we are expanding our team.

With 148 beds, serves a diverse population of short-term rehab patients, long term care residents and memory care.  We have:

  • An active therapy dept. achieving terrific functional gains and a remarkable restorative program
  • We have a stable team, with a DON that has been with the center for years

Job Description

About you:

The Nursing Home Administrator (LNHA) sets the pace for the skilled nursing facility.  You lead by example and you know first impressions matter.  People remember you, respect you and want to work with you.  You make an impact in your patent’s lives and you strive to make a difference in your center's metrics. 

  • Your number one goal will be to increase marketing initiatives & focus on rebuilding census.
  • This will involve interfacing with our union and representing the center at county board meetings. 

Qualifications

What we will need from you:

  • Currently hold the Illinois LNHA license
  • Prior experience (3+ years) as an LNHA in a skilled nursing facility
  • Ability to build a team and potentially bring some with you
  • A love for what you do, and your education and credentialing reflects that
