Our center:
Crestwood Terrace is an Intermediate Care Facility (ICF) with 126 beds and is providing patient care to the mentally ill/behavior population (ages 21 and up).
Location: This beautiful spot is across the street from a forest preserve and only 3 minutes from the 294/Cicero exit.
Job Description
MDS Coordinator / Registered Nurse (RN) - officially we title this job an MDS Coordinator (RN), but in other centers it may be called a Director of Resident Assessment, Case Manager, RN, or Registered Nurse.

Qualifications
We we need from you:
- MDS 3.0 /RAC certification
- Registered Nurse (RN) license in IL
- 2+ years’ experience in a SNF or ICF setting as an MDS Coordinator or Director
- Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement
- Thorough understanding of the Quality Indicator process
- Knowledge of the OBRA regulations and Minimum Data Set
