Under new leadership:  Transitional Care Management (TCM) now manages this Intermediate Care Facility (ICF) as of 9/1/2019.  Our goal is a focus on quality care and excellent customer service for the staff & the patients alike.  While we have many team members that have been with the center for 10-15 years, we are expanding.

More about the job:

Under the guidance of the DON, you will be leading the restorative and wound care programs for the facility.  You will provide oversight for all the CNAs in the center – supervision, management and staffing.  This ADON will also be responsible for leading the falls prevention and pain management programs.

What we need from you:

• 3-5+ years of experience as an ADON, Charge Nurse or Nursing Supervisor in an ICF or SNF setting
• Registered Nurse (RN)
• Experience leading:  restorative, wound care, falls prevention and pain management
• Strong CNA leadership experience
