Our facility:  Thrive of Fox Valley

This brand new 68 bed facility dedicated to the short term rehab patient.  This is NOT a long term care facility.  The average length of stay will be 4-14 days and we will have 5-10 admissions per day.  Most of our “guests” will be joining us after an orthopedic surgery, cardiac event or after a medically complex stay in a hospital.  This is a rehab and recovery focused facility with a large therapy team of in-house therapists.  We are here to bridge the gap from hospital to home, and make it a smoother, safer, transition on the road to recovery.

This is a busy facility and we are looking for an MDS Coordinator experienced with high volume admissions/discharges.

Job Description

MDS Coordinator

  • Registered Nurse (RN)  
  • Start date:  Sept or early October
  • Salary:  based on prior experience & credentials
  • Status:  full time days with an every other weekend rotation

Position overview:

The MDS Coordinator will work under/with our MDS Director to coordinate and oversee the interdisciplinary assessment and care planning process in our 100% short term (rehab) setting. This process includes comprehensive resident assessments; care coordination and planning; resident advocacy and teaching; facilitation of open communication among care team members, the resident, and family; collection and transmission of data for the purposes of quality improvement; and adherence to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements. The ultimate goal of the assessment and care planning process is to promote the resident’s quality of care and satisfaction in our center.

  • Prior experience as an MDS Coordinator in a rehab setting (SNF) is a must.


  • Registered Nurse (RN) license in Illinois
  • Current MDS 3.0 certification
  • Minimum of 2 years of MDS experience in a short term unit within a Skilled Nursing Facility
