About the facility:

Under new leadership:  Transitional Care Management (TCM) now manages this Intermediate Care Facility (ICF) as of 9/1/2019.  Our goal is a focus on quality care and excellent customer service for the staff & the patients alike.  While we have many team members that have been with the center for several years, we are expanding.

Job Description

This is an entry level CDM position.

Overview:

You have the ability to impact our patient’s lives on a daily basis by influencing the nourishment we provide them. This is an important role that we take seriously. Providing a home like environment includes the provision of tasty, nutritional and appealing food. It can impact their recovery and their daily state of mind.

Qualifications

Requirements:

  • This is an entry level CDM position. 
  • Prior experience as a Director is not necessary, but you must hold the Certified Dietary Manager credentials
  • ServSafe Certifications

Essential Functions

  • Direct management and supervision of food services managers and dietary staff
  • Understands nutritional requirements for entire population and menu options available
  • Reviews and approves Menus (regular and therapeutic) and substitutions per policy
  • Completes nutritional assessments on residents within required timeframes and updates as necessary
  • Attends Care Plan meetings per policy (as needed)
  • Monitors weight gains/losses and determines interventions for those at nutritional risk (as needed)
  • Makes recommendations on patient’s nutritional needs to nursing management and physicians
  • Audits meal services to assure compliance with systems, policies and procedures
  • In-services staff as indicated (Cooks and all staff as needed)
  • Ensures Proper storage and handling of food in center
  • Temperature logs are maintained for all equipment
  • Personal hygiene practices in place
  • Food safety and correct temperatures are maintained
  • Other duties as assigned
