Job Description

The Clinical Support Specialist will be responsible for reviewing all new referrals to ensure the patients' care coordination (clinical, geographic, insurance, etc.) needs are met. This position will also play a vital part in providing support to the Quality Assurance (QA) and Intake Departments.

Key Responsibilities are:

  • Review all Start of Care (SOC) and Resumption of Care (ROC) referrals
  • Utilize insurance portal to verify home health benefits for all referrals
  • Run requested reports weekly or as needed
  • Utilize hospital EMR portal to pull patients' most recent medical records, or send request via fax to verify re-hospitalization admission date and diagnosis
  • Assist with retrieving clinical documentation from EMR system for the clinical department. Documents will be downloaded from the EMR system and given to the clinical department via email
  • Attend mandatory staff meetings, care conferences, and education offerings
  • Assume the responsibility for person growth through continuing education
  • Other assigned duties as needed

 

Qualifications

Medical Assistant Certificate

Minimum of one (1) year in a clinical setting or a medical practice; preferably in a home care setting

Ability to work through interruptions and multi-task

Ability to work independently and as part of a team
