Job Description

Responsible for ensuring that the Patient Tracking Log is updated daily.

Responsible for ensuring that accurate completion of Referral Worksheets.

Responsible for entering patient information for all new patients into computer software ensuring speed and accuracy during the process.

Responsible for adding new physicians into computer software after verifying that the physician is in the PECOS system.

Responsible for uploading legible referral documents into computer software on a timely basis.

Assigns and coordinates all patient services and assist the clinical staff to ensure efficient delivery of quality care with the assistance from ADON or DON.

Collaborate with staffing agencies to ensure proper scheduling for all patients.

Utilize telephone, texting, and electronic communication to ensure effective communication with field staff.

Verify that all visits scheduled correspond with the ordered frequency.

Responsible for giving report daily to the administrative on call nurse with any scheduling issues that have not been resolved during working hours.

Ensure appropriate supervisory notes are scheduled (HHA, PTA, COTA).

Responsible for monitoring the Discharges and Re-certifications to ensure proper forms are in the system on a weekly basis.

Demonstrates knowledge of Medicare, Medicaid, and insurance industry guidelines.

Maintains active knowledge of home care regulations and standards, updates, and changes.

Ensures available resources are utilized appropriately by providing information to telephone callers regarding services available through the Agency and/or the community.

Performs a variety of clerical work in an independent manner, exercising judgment and initiative; speed and accuracy.

Responsible for verifying primary care physician to ensure we have a physician to follow our patients after our initial order for home care.

Responsible for relaying any pertinent clinical details of referrals to field staff via telephone, text message, and/or electronically.

Performs a variety of intake/scheduling processes in an independent manner, exercising judgment and initiative; speed and accuracy.

Demonstrates knowledge of Medicare, Medicaid, and private insurance industry guidelines.

Responsible for maintaining competency and proficiency in all computer software programs in use by the Agency.

Ensures available resources are utilized appropriately by providing information to telephone callers regarding services available through the Agency and/or the community.

            Learns, interprets, and applies organizational policies, rules and regulations.            

            Proofreads typewritten material to verify accuracy and coherence of content, using                                    knowledge of medical terminology and other references when necessary.

Maintains confidentiality of all patient information in work area.            

Uses all Agency equipment appropriately and maintains optimal conditions to prevent damage of equipment.

            Performs other duties and participates in projects as assigned.

Qualifications

Graduate with a minimum of a High School Diploma.

 Minimum of 2 years’ experience as an Intake/Scheduling Coordinator in a Health Care Setting, preferably Home Health Care.
