Job Description

Are you a Registered Nurse with extensive knowledge in MDS? Are you well-organized and very detailed orientated? Are you confident in your training and abilities and are able to communicate positively and effectively with individuals at all levels?

As the MDS Coordinator, you are a valuable member of the leadership team. You are responsible for overseeing and facilitating the completion of resident assessments.  You set assessment schedules and assure that they are completed in an accurate and timely manner. You coordinate the care plans for patients according to regulatory requirements to help meet the short term and long term healthcare goals of the resident. You have a good understanding of Medicare and Medicaid. You remain up to date on Medicare coverage and help determine documents needed for newly qualified patients.

Qualifications

 

  • A Registered Nurse in the State of Indiana with at least one year of experience in a long term care setting
  • Proficient in MDS
  • Passionate about delivering excellent customer service
  • Professional in appearance and behavior
  • Able to work with a dedicated team
