Job Description

1.  Effectively communicates with physician offices and families when needed.

2.  Maintains active knowledge of home care regulations and standards, updates, and changes.

3.  Learns, interprets, and applies organizational policies, rules, and regulations.

4.  Interpret Physician Orders and other medical record documents to verify accuracy and coherence of content, using knowledge of medical terminology and other references when necessary.

5.   Maintains file folders, file correspondence and reports in accordance with established instructions.

6.   Tabulates statistical reports. Assists Clinical Managers with maintaining reporting as needed by DON, ADON, Regional Directors, and CEO as needed.

7.  Ensures available resources are utilized appropriately by providing information to telephone callers regarding services available through the Agency and/or community.

8.    Collaborates with QA Clinical Manager and DON to assure quality of patient care.

9.    Assists QA Manager & Coder with completion of the tool of OASIS-D SOC

10.  Assists QA Manager & Coder to QA OASIS documents (SOC/ROC/Recert).

11.  Assists QA Manager to assign SOC orders, flag charts to identify various diagnoses, send care coordination information to staff, and verify correct frequencies.

12.  Assists QA Manager in approving therapy evaluations.

13.  Assists QA Manager to verify appropriate scheduling of therapy 30 day evaluations & PTA/COTA/HHA supervisory visits.

14.   Uses all agency equipment appropriately and maintains optimal conditions to prevent damage of equipment.

15.   Maintains confidentiality of all patient information and work areas will be kept to maintain confidentiality of patient information.

16.   Performs other duties and participates in projects as assigned by supervisory staff.

17.   Advises the QA Clinical Manager, DON, and/or Administrator of any current events, problems, and agency operation concerns on an as needed basis.

18.   May complete home visits if necessary based on staffing needs.

Qualifications

Educational Requirements:          

Graduate of an accredited practical or vocational nursing program. Licensed to practice in the State of Indiana.

Experience:                                       

Minimum of one (1) year nursing experience in Med-Surg required. Home care nursing experience preferred. Knowledge of office, procedures, filing, and record keeping principles and practices. Good communication and organizational skills.

Qualifications:

Must have computers skills and the ability to work EMR Personal appearance neat, clean and professional.

Demonstrates organizational skills and ability to work within a team concept.

Possess interpersonal and communication skills.

Analytical approach with data analyzes for quality assurance. Demonstrates efficient time management skills.
