Job Description

A day in the life at Wonderful

  • Responsible for conducting an intake and all required paperwork for program enrollment.

  • Responsible for completing comprehensive assessments in the areas of health, nutrition, psychosocial needs, education/vocation, goals, parenting skills, and home safety for each client.
  • Responsible for maintaining an assigned caseload
  • Monitors planned actions by periodic follow-up and case review with FRC Social Worker.
  • Keeps accurate, up-to-date records on clients served including contact summaries, progress notes, support service plans, and program-specific reports.
  • Works with other staff to develop community resources.
  • Serves as liaison with other community agencies, schools, and department initiatives.
  • Obtains assistance for the client by referring him/her to community resources, arranging for appointments, and establishing rapport with other agencies.
  • Maintains records of the case by documenting the client's situation and the client's actions.
  • Order and organize essential supply inventory
  • Maintains client confidence and protects operations by keeping information confidential.
  • Advocates for clients when there is a problem in the service delivery system.
  • Provide logistical support and community outreach.

Qualifications

Your contribution to Wonderful:

  • Bachelor's degree (BS or BA) from 4-year college or university preferred

  • 3+ years providing community-based programming and case management

  • Must be able to read, write and speak Spanish without the use of an interpreter

  • Ease working with minimal supervision, while taking the initiative to ask for guidance if needed

  • A positive, thoughtful team player who is ready to roll up their sleeves

  • Professional appearance and excellent attitude

  • Proficiency in Microsoft Office Suite; Word, Excel, and Outlook.

  • Excellent organizational, time management, and multi-tasking skills with attention to detail

  • Comfortable in a fast-paced, dynamic environment

  • Fluency in Spanish is required

  • COVID vaccination will be required for this position unless the candidate has a legally valid exemption
