Job Description

The Memory Care Coordinator functions as the main point of contact for residents, families, and staff in our Footsteps Memory Care. The MCC must be an excellent communicator as they need to ensure all parties are appropriately informed. In this position, you will work closely with the community RN to ensure the accuracy of care for our treasured residents. 

Our ideal candidate will serve as the primary contact for residents and their families. As the Memory Care Coordinator you are responsible for monitoring alert charting and communication logs, assuring Health Services employees are properly informed of changes in care, scheduling of appointments with physicians, and other health-related appointments for those we serve.

Other responsibilities include oversight of service planning, maintaining our memory care license, assistance with employee scheduling, and employee development. 

Qualifications

  • At least 18 years of age
  • Excellent attendance and flexibility to work as needed.
  • Prior experience working with senior populations with dementia
  • Prior experience working in senior living, preferably involving medication administration
  • Strong computer skills. Ability to maintain focus in a fast-paced environment
  • Excellent interpersonal and team-building skills
  • Ability to pass criminal background and pre-employment drug test
  • Sincere appreciation for elders and observable respect for caregivers, med techs and the important work they do.
  • 3-5 years Health Care experience
  • Ability to effectively present information and respond to questions from groups of managers, residents, employees, and the general public.
  • Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations
  • Ability to write reports, business correspondence, and procedure manuals. · Ability to prioritize, organize, and delegate assignments.
  • MC regulations knowledge
  • Ability to interpret a variety of instructions furnished in written, oral, electronic, or schedule form.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Proficient PC skills including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.
