Overview

A large acute care hospital in Riverside, CA is hiring a Manager of Case Management to lead a dynamic care coordination team. This is an ideal role for a results-driven RN with proven leadership in hospital case management who can hit the ground running and drive throughput, discharge planning, and compliance initiatives. The ideal candidate brings both operational oversight and clinical insight to improve patient outcomes and ensure continuity of care.

Job Description

Responsibilities

  • Manage day-to-day case management operations, including scheduling, attendance, and staff development

  • Drive throughput and facilitate timely discharge planning to avoid delays in patient progression

  • Provide leadership to RN Case Managers and Assistants (15–18 FTEs per manager)

  • Serve as a key escalation point for post-acute discharge barriers, transportation delays, HLOC transfers, and service recovery

  • Engage in hospital-wide care coordination and ensure compliance with federal, state, and payer-specific regulations

  • Educate staff on processes related to care coordination, MCG/InterQual criteria, discharge planning, and reimbursement

  • Collaborate with physicians, insurance providers, and families for proactive planning and expectation setting

Qualifications

  • Required: BSN; Preferred: MSN

  • California RN license (will consider pending license)

  • Minimum 3–5 years acute hospital case management experience and leadership experience

  • Strong understanding of post-acute care criteria, payer requirements, and reimbursement frameworks (Medicare, Medi-Cal, managed care)

  • Proficiency in InterQual and MCG guidelines

  • Proven ability to lead in high-acuity inpatient environments and address barriers to discharge

Additional Information

Schedule & On-Call

  • Typical Hours: Monday – Friday, 8:00 AM – 5:00 PM (exempt position)

  • On-Call: Rotating weekly within leadership team; weekend call involves issue resolution, authorizations, transportation, and team support
