Job Description

Ensures customers are directed to the most appropriate community resources to meet their needs. Provides information to community partners to support the ongoing distribution of resources based on local needs.

1.Receives referrals and determines eligibility and enrolls customers into appropriate programs with the intake process.

2.Assesses customer needs through comprehensive assessments.

3.Makes referrals and encourages customer to take advantage of relevant programs, in order to enhance their self-sufficiency and meet their immediate needs.

4.Coordinates services through case management. Follows up with customers to determine use of resources and/or need for referrals to additional resources.

5.Maintains an accurate, current and comprehensive regional resource data base of health, human service and other community-based organizations’ service information.

6.Disseminates updated and relevant marketing about Regional Resource Collaborative services to community-based organizations that may provide financial and/or other types of assistance of potential interest to program customers.

7.Initiates and develops strategies to enhance inter-agency collaboration to better serve customers.

8.Maintain individual customer files.

9.Analyze data and develop reports for dissemination.

10.Participate in task force meetings and other related meetings or trainings.

11.Actively engages in staff meetings and conference calls.

12.Participate in professional development activities.

13.Other duties as may be assigned by supervisor.

Qualifications

Education and Experience Requirements:

•Minimum of an Associate’s Degree with three years of experience.

•Bachelor’s degree with one year experience preferred.

•Extensive knowledge of community resources and services.

•Demonstrated ability to work with volunteers and community organizations.

•Experience providing presentations to groups.

•Must have own transportation, a valid driver’s license, and vehicle insurance.

 

Skills and Abilities:

•Microsoft Office products and on-line client management systems.

•Bilingual ability preferred.

•Problem-solving, arithmetic calculations

•Ability to effectively communicate in a professional manner, both orally and in writing

•Ability to de-escalate and handle high stress situations

Additional Information

•Work is performed mostly in an office setting.

•Subject to long periods of sitting and exposure to computer screen.

•Keyboarding and data entry, use of telephone.

•Occasional exposure to adverse weather conditions when working in community.

•Sitting, walking, stooping, squatting, lifting (up to 25 pounds), carrying, pushing.
