Spring Creek Rehab and Nursing is seeking motivated and compassionate Licensed Practical Nurses to join our team!

We have Full time and Part-Time shifts available.

Day shift 7am-3pm -part time

Evening Shift 3pm - 11pm - full time and part time

Night Shift 11pm-7am -full time and part time

LPN with trach and vent exp - 12 hour day shift and 12 hour night shift

The LPN is responsible for the following:

• Monitoring residents, their responses to treatment care plans.
• Supervising and directing.
• Document necessary information regarding nursing care, care plans and observation of residents overall conditions.
• Perform various duties to provide quality nursing care to residents.
• Analyzes information for appropriate interventions to prevent decline.
• Record signs of change in condition.
• Notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.
• Administers medications and treatments to residents; monitor administration of intravenous fluids; places orders for medication.
• Maintain resident clinical files; keep charts updated; document appropriately.
• Make rounds to see each resident daily.

Job Requirements

• At least one year of experience in a Long Term Care environment is highly preferred.
• Must be a graduate of an approved practical nursing program.
• Must possess and maintain a current state PA license as a Licensed Practical Nurse (LPN)
• Must be able to move, lift and position patients.
