Job Description

Social Worker  will be Lic. MSW in Texas , with at least 3+ years of experience.  Social Worker will  will be responsible for:

 • Performing discharge assessments on the floor and in the Emergency Department •

 Identifying High-Risk patients and assessing their potential needs •

Working with a multi-disciplinary team within hospital and outside of hospital to secure services and referrals for patients • Following up with transitions patients who may readmit to the hospital and completing a discharge assessment, including a readmission assessment

• Communicating daily via email, phone, or in person with the Transitions of Care NP and follow up with patients who may be in-house or in the emergency department

• In addition to seeing transitions patients, this Community Connections Social Worker (CC-SW) will see patients in the Emergency Department daily that have been identified as in need of service. This can be a referral from the physician, nurse, UR nurse, or the patient and/or patient’s family or the CC-SW own review of records.

Qualifications

• Master's degree (M.S.W.) from four-year college or university. • Minimum of 2 years’ experience in an acute care or community health setting • Strong leadership and excellent interpersonal skills to effectively coordinate and coach other to deliver care in a coordinated, efficient and effective manner

Current SW certificate
