About the role
You will play a key role in coordinating and guiding clinical care for our clients across the community. Reporting to the Clinical Team Leader, you’ll provide expert advice, manage clinical escalations, and ensure best practice care is delivered consistently. You will work closely with clients, families, and internal teams remotely to develop care plans that promote independence, health literacy, and quality of life. This is a dynamic role that combines clinical work, coordination, and strategic input.
You will be required to regularly attend our local office in Sydney and on rare occasions, complete hands-on clinical work.
Key responsibilities
- Deliver high-quality clinical care coordination, including assessment, re-assessment, referrals, and reablement interventions.
- Provide clinical advice and guidance to operational teams.
- Support onboarding of new CHSP clients and manage My Aged Care portal entries.
- Conduct medication management reviews and health information assessments.
- Liaise with GPs, allied health, and other healthcare providers
- Promote health literacy and self-care among clients.
- Contribute to continuous improvement and best care practices.
What we’re looking for
- Current AHPRA registration as a Registered Nurse.
- Tertiary qualifications in a health-related discipline.
- Experience in aged care or community health (highly desirable).
- Strong clinical assessment and care planning skills.
- Excellent communication and stakeholder engagement abilities.
- Ability to work independently and manage a caseload.
- High level of computer literacy and comfort with digital systems.
- A proactive, values-led approach with a focus on continuous improvement.
- Current driver’s licence, reliable vehicle, and appropriate insurance.
- Willingness to travel within the community as required.
