Job Description
- Provide clinical pharmacy services in accordance with professional standards and hospital policies.
- Review prescriptions and medication orders to ensure the appropriateness of drug selection, dosage, route, frequency, and potential drug interactions.
- Conduct medication reconciliation and medication reviews as required.
- Monitor the effectiveness and safety of drug therapy and identify potential Drug-Related Problems (DRPs).
- Provide evidence-based recommendations regarding drug therapy to physicians and other healthcare professionals.
- Monitor and report adverse drug reactions (ADRs), medication errors, and other medication-related problems.
- Provide medication counseling and education to patients and their families.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes and ensure medication safety.
- Ensure accurate and timely documentation of clinical pharmacy activities.
- Participate in quality improvement and patient safety programs related to medication management.
- Perform other duties related to clinical pharmacy services as assigned by the supervisor.
Qualifications
- Master’s Degree (S2) in Clinical Pharmacy from a recognized university.
- Possess a valid Pharmacist Registration Certificate (STR).
- Possess a valid Pharmacist Practice License (SIP) or meet the requirements to obtain one.
- Previous experience in hospital clinical pharmacy is preferred.
- Strong knowledge of clinical pharmacy practice, pharmacotherapy, medication safety, and patient care.
- Strong analytical, communication, and interpersonal skills.
- Able to collaborate effectively with physicians, nurses, and other healthcare professionals.
- Detail-oriented, proactive, responsible, and patient oriented.
