Senior CommUnity Care of NC, sponsored by Volunteers of America, is a Program of All-Inclusive Care for the Elderly (PACE). We provide healthcare services to the frail, elderly who need nursing home care but want to stay in their homes.

Job Description

ESSENTIAL FUNCTIONS:


1. Conducts or coordinates an in-person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive care plan for each participant.


2. Conducts or coordinates an in-person nursing re-assessment semi-annually and as needed.


3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on plan of care planning decisions and coordination for twenty-four (24) hour care delivery.


4. Provides orientation, initial and annual competencies, supervises and evaluates performance of Clinical Registered Nurse (R.N.), LPN,  Health Care Coordinator, and Clinic CNA


5. Assisting with recruiting and selecting nursing staff.


6. Performs competencies on appropriate Day Center staff as needed.


7. Implements systems to assure compliance with all state and federal and local laws and regulations.


8. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Ensures that all direct reports works within their scope of practice.


9. Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinic staff to function independently.


10. Oversees staffing and scheduling for the nursing departments and ensuring appropriate staffing levels to include on-call scheduling and coverage.  


11. Evaluates the overall effectiveness of the reporting departments. Implements change and quality improvement as needed.


12. Manages flow of clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensures timely and appropriate care of participants.


Qualifications

1. Education: Graduate of a school of professional nursing program required. Bachelors of Science Registered Nursing (BSN, RN) strongly preferred. Current Registered Nurses License, Cardio Pulmonary Resuscitation (CPR) certification and first aid required.


2. Experience: A minimum of one year of experience working with the frail or elderly population in an outpatient clinic setting and home care experience within the last three (3) years preferred. At least two (2) years of nurse management experience in a hospital, nursing home, home care, outpatient clinic or community-based setting preferred.

 

3.  Skills/Knowledge:

• Management and leadership skills.

• Broad knowledge base of health care problems of the frail elderly.

• Skilled in physical assessment and triaging.

• Knowledge of medical equipment and instruments.

• Knowledge of common safety hazards and precautions to establish a safe working environment.

• Skilled in identifying problems and recommending solutions.

• Skilled in preparing and maintaining records and writing reports and responding to correspondence.

• Skilled in developing and maintaining department quality assurance.

• Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.

• Ability to react calmly and effectively in emergency situations.

• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
