Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Description

Job Summary

The Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care under the Culver City FSS contract as well as adhering to the mission of St. Joseph Center. The Case Manager provides case management services leading to family self-sufficiency for families residing in Section 8 housing within the City of Culver City.

ESSENTIAL DUTIES & RESPONSIBILITIES T

Key Areas of Responsibility:

  • In conjunction with the City of Culver City, facilitate orientation, intake and assessment for new families enrolling in the Family Self-Sufficiency program.
  • Provide ongoing case management for up to 25 families. Identify barriers to self-sufficiency and, along with the family, develop strategies to address these barriers. Special attention will be paid to job readiness, job training, identification of childcare resources and other activities leading to employment and self-sufficiency.

Essential Duties:

  • Along with the family, develop an Individual Treatment and Service Plan (ITSP) in keeping with HUD guidelines.
  • Document all activities and outcomes as required by the City of Culver City and HUD.
  • Assist with the collection of data and the preparation of program reports and annual surveys.
  • Participate in the coordination of activities for the participants of the FSS program, including facilitation of program events and scheduling of program activities (such as budget workshops and employment networking events).
  • Conduct phone screenings, assessment services, and in-person appointments. Recruit and enroll participants in the Family Self-Sufficiency (FSS) Program by completing a HUD Contract of Participation.
  • Maintain client files, case management sessions and community resources. Establish and build ongoing relationships with colleagues, housing authority and provider agencies.
  • Manage documentation of case notes and service transactions in Happy software system to ensure HUD compliance. Compose and prepare correspondence, and summaries of participant data.
  • Assist clients and/or their families in achieving Individual Training and Service Plans (ITSP) goals, developing life skills, and/or sustaining financial stability. Monitor participant progress activity. Help participants confront barriers and stay on track with accomplishing goals successfully.

Qualifications

QUALIFICATIONS

Knowledge, Skills & Abilities:

  • Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent administrative and organizational skills.
  • Working knowledge of labor market, job training and placement resources.
  • Bilingual Spanish/English desirable.

Experience:

  • One to two years of experience providing case management services in a community setting with homeless or at risk population.

Education:

  • Bachelor degree from an accredited college or university in social work, human services or a related field.

Direct Reports:

  • This position has no direct supervisory responsibilities.

Computer Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.

Certificates, Licenses and Registrations:

  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Valid automobile liability insurance.
