• Oversight and care management of in-home client care
• Supervision of in-home care staff
• Ensure compliance with IDPH and DOL regulation
• Working with Director of Marketing to help market skilled nursing services to local community
• Fill-in for occasional in-home skilled nursing needs

OVERSIGHT AND CARE MANAGEMENT

• Serve as liaison between clients/client families and the office
• Conduct new client assessments and develop client care plans
• Oversight of client care through in-home visits and regular phone call check-ins.
• Guide clients/client families when making decisions about care
• Document all observations and interactions

SUPERVISION

• Supervise in-home staff, including CNAs, caregivers and nurses to ensure proper care
• Communicate care plan to in-home staff
• Provide guidance and on-the-job training as needed to ensure care plan needs are being met by in-home staff
• Provide timely feedback to in-home staff, including disciplinary measures as needed
• Document all observations and interactions

COMPLIANCE

• Ensure all hiring paperwork is complete and compliant with Department of Labor and Department of Homeland Security Regulation
• Ensure all background checks, etc., are conducted in compliance with IL Dept of Public Health
• Maintain up-to-date employee paperwork
• Update policy to comply with Joint Commission in pursuit of accreditation

OUTREACH

• Working with marketing department to help formulate strategy to build skilled nursing services
• Attend occasional marketing and networking events
• Visit long-term care facilities to help strengthen relationships
