Job Description

Are you tired of being held to a productivity model of Psychiatric Care? If you are seeking a work environment that fosters collaboration with patients, families, primary care providers, and clinical therapists, that allows you 60 min visits with all your patients (across the lifespan), and competitive pay, we have an opportunity for your consideration. 

We are looking for candidates interested in 2 - 3 days a week between 2 locations . No more than 8 patients are booked per day you are on site. Non-direct care time is for collaboration with PCP, Integrated Behavioral Health Clinicians, and other specialty providers at the sites.  There are 2 clinic locations, the first is near 35E and Hwy 96 and the second is off 494 and Dodd Road (Eagan and White Bear Lake).

Both sites are part of the same system of Primary Care Clinics which we have been partners with for over 3 years. You will have access to the clients full medical record as we are integrated into the partner clinics EMR. Labs are on site, medication dispensary is also on site (most common psychiatric medications are available free to the members eligible to utilize the clinics), Pharmacogenomic testing (we will train you on interpretation if needed) is fully covered by the patients plan. 
As a PMHNP with Praestan Health you will not be providing Treatment As Usual (TAU), you can expect to...

  • Work in a primary and specialty care clinic, serving as a Praestan ambassador in those environments On-site 
  • Develop trusted relationships with Primary Care and Integrated Behavioral Health Providers while promoting a whole-person approach to patient care.
  • Provide a comprehensive, direct services to adult, adolescent, and child psychiatric and substance use concern patients.
  • Provide assessments including physical and psychiatric functioning and pharmacological interventions in the outpatient setting, diagnostic interviews and individual, family, and group therapy services to patients and their families.
  • Regularly engage and collaborate with other providers on case review and treatment plans.
  • Maintain the ability to provide 2 psychiatric comprehensive assessments and 6 follow-up encounters or Collaborative consultations with Primary Care providers per day worked.
  • No evenings, no weekends, no holidays 

Qualifications

  • A current MN license as an APRN with prescriptive authority and ability to obtain and/or maintain DEA certification with the State of Minnesota
  • Must be a graduate of an accredited school of nursing with Psychiatric-Mental Health Nurse Practitioner (Across the Lifespan) and board certified through the ANCC
  • Priority given to applicants who have a Doctor of Nursing Practice (DNP) degree
  • Previous experience as a Psychiatric-Mental Health Nurse Practitioner is preferred
  • Interest and ability to work within our clinical model and therapeutic approach, while also working independently within a partner primary or specialty clinic setting.
  • Ability to clearly summarize pertinent clinical information via written correspondence and medical record documentation
  • Strong skills in case management, time management and both written and verbal communication
  • Ability to comply with HIPAA regulations
  • Ability to maintain required malpractice insurance
  • Oriented and educated in gender, culture and age-appropriate care
  • Strong oral and written communication skills
  • Proficient in Microsoft Office products including Word and Outlook
