Job Description

The Patient Support Specialist (PSS) serves as Praestan Health’s Engagement and Care Coordinator.  The focus of the role is on ensuring identified patients receive access to the care they need and are engaged with Praestan’s Mental Health and Wellbeing services, including coaching, therapy, primary care, or psychiatric care.

As a PSS you will

1)    Be the first point of contact for the patient and identified care team members to ensure coordination and access to care

2)    Encourage and assist patients to engage with the care team and PortagesTM to improve the patents mental health and wellbeing

3)    Attend care team meetings to provide insights and assistance with the collaborative care approach

4)    Guide patients with enrollment and registration into the Portages program

5)    Provide appropriate referrals for levels of care based on the Portages  model

6)    Access and document in Electronic Health Records to communicate valuable information to the care team

7)    Assist patients with implementing technology into their health care

8)    Liaison between Praestan and Glencoe Providers to ensure coordination of care

9)    Support on site Praestan Clinicians with scheduling and engagement projects

10) Ensure that the  PortagesTM Program is delivered with high quality and  fidelity to the model

Qualifications

Highly Qualified Candidates will

1)    Have a Bachelor degree in a human services field (psychology, sociology, social work, or medical).

2)    A desire to support individuals, families, and care givers in their mental health and wellbeing.

3)    High degree of flexibility and the ability to adapt in a fast pace environment

4)    An optimistic disposition and team centered view of patient care

5)    Excellent organizational, interpersonal, and customer service skills

6)    Proficient computer skills in Microsoft Office including Word, Excel, and Outlook

HIPAA Minimum Necessary Access Level: Level II – Access to records as necessary for software development.
