Job Description

 Our IBH Clinical Therapists leverage our proprietary technology and analytics platform, along with our innovative assessment and clinical tools, to partner with patients in creating targeted mental health and wellbeing care. Clinical Therapists and patients, families, and medical providers then work together over a brief period of time (an “episode of care”) to achieve these objectives.

As an IBH Clinical Therapist, you will:

·       Work with psychiatric care providers, patient support specialists, coaches, physicians, and other members of medical care teams to evaluate patient mental health and wellbeing, and develop comprehensive care plans

  • Work with adult and pediatric populations with Mental Health concerns, Substance Use Disorders and Co-occurring conditions
  • Utilize the Praestan Model of care and Portages outcome data to support treatment planning and therapeutic interventions
  • Work with Individual, Couples, and Families to achieve their goals of wellbeing
  • Provide clinical diagnostic assessments, referral triage, primary and/or specialty care provider consultation and treatment monitoring
  • Provide targeted psychotherapy and wellbeing counseling aligned with targeted biopsychosocial treatment plans
  • Work inside primary and specialty care clinics as a memeber of the care team, serving as a Praestan Health ambassador developing trusted relationships with providers and promoting a whole-person approach to patient care  

Qualifications

Education and/or Experience:  Completion of an accredited program with certification and Minnesota licensure as a Marriage and Family Therapist, Professional Clinical Counselor, Independant Clinical Social Worker, or Psychologist.

License and / or Certification:

LMFT, LICSW, LP, LPCC applicants with couples work background is preferred.

At least two years of experience in mental health counseling required.

  • Valid Minnesota Drivers License  
  • Higher preference placed for applicants with graduate training in Substance Use Disorders and Co-Occurring Disorders
  • Ability to provide 25 patient encounters per week over 5 days
  • Experience working within a primary care or integrated healthcare setting, or health coaching experience in primary care clinics preferred, but not required
  • Ability to work one evening a week until 6pm one day a week
  • Ability to document in an EMR, ideally during patient encounters
  • Interest and ability to work with our clinical model and therapeutic approach
  • Interest and ability to work independently within a partner primary or specialty care setting
  • Ability to maintain require malpractice insurance
  • Oriented and educated in gender, culture and age appropriate care
  • Strong oral and written communication skills
  • Proficient in Microsoft Office products including Word and Outlook
