Job Description

As a Praestan Health Outpatient Clinic Psychiatric Care LPN, you will:

  • Check-in adult and pediatric patients, make reminder calls, and schedule appointments
  • Collect insurance cards/information
  • Send/receive medical record requests, letters, outside agency forms, etc.
  • Conduct pre- and post-visit checks on patient charts and treatment plans
  • Serve as clinical team liaison to patients, internal team member and partner primary care clinic team members
  • Provide introduction information to patients about Praestan Health services and coordinate information obtained from screening tools
  • Provide basic elements of health coaching and engagement outreach to patients under the discretion of the Coach Therapist and Psychiatry Provider and as aligned with the patient's treatment plan
  • Schedule interpreters for patient appointments as needed
  • Room patients for appointments
  • Complete blood pressure, height, weight, pulse, temperature checks, and urine testing if needed prior to appointments
  • Initiate referrals to internal team members (psychiatry and psychological testing)
  • Write patient informational letters and complete patient information forms as needed
  • Serve as the extension of the psychiatry provider to the patients and the primary care team and providing positive supports
  • Primarily office out of our primary care partner clinic in Eagan, but travel to clinic sites in White Bear Lake, Maple Grove and Minneapolis will be intermittently necessary

Qualifications

  • High school diploma required with completion of 2-year LPN program with certification required
  • Knowledge of medical and medication terminology
  • Must have critical thinking skills, problem-solving ability, and to communicate and make safe judgment calls in stressful situations
  • Must be able to multi-task
  • Excellent organizational, interpersonal, and customer service skills
  • Proficient computer skills in Microsoft Office including Word and Outlook
  • Must be able to drive own vehicle between clinic sites across the Twin Cities metro area

FLSA Status: Non-Exempt

HIPAA Minimum Necessary Access Level: Level III, access to records for scheduling appointments, authorization and billing.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Lifting/carrying occasionally up to 25 pounds.  This category usually requires frequent bending/stooping.  Standing/walking is generally required off and on for a total of approximately 6 hours of an 8-hour workday.  Sitting may occur intermittently during the remaining 2 hours.  In most medium jobs, being on one’s feet for most of the workday is critical.  Medium jobs require use of the hands and arms to grasp, hold and turn objects but generally do not require the use of fingers for fine activities to the extent required for sedentary work.  Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

OSHA Category II:  This category involves workers who perform tasks that involve minimal exposure to blood, body fluids, or tissues, but exposure or potential exposure may be required as a condition of employment. Appropriate protective measures will be readily available to every employee engaged in Category II tasks.
