Positive Solutions Behavior Group is a family-focused resource that continuously strives to increase opportunities for individuals with autism spectrum disorder and other developmental needs by applying the principles of Applied Behavior Analysis (ABA) in the community, home, schools, and clinical setting. We recently expanded our operations to include Targeted Case Management and are seeking a full-time Targeted Case Manager to provide services in the Northern Kentucky area to recipients seeking mental health services.

Job Description
-Assess, plan, implement, monitor, and evaluate client’s health and needs.
-Handle case assignments of up to 25 clients.
-Create and implement treatment plans.
-Help clients make informed decisions and advocate for client.
-Provide education and support to client and client’s family.
-Gather and make appropriate referrals to client.
-Prepare and maintain client records and notes in data collection system.
-Attend necessary trainings and certification courses.
-Ability to travel locally up to 50% of time.
-Participate in networking activities and related community organizations.
-Other duties as assigned.

Qualifications
-Bachelor’s Degree in a human services field.
-Targeted Case Management Certifications or the ability to complete Targeted Case Management Certifications within 1 month of hire date.
-Excellent interpersonal skills and ability to work effectively with team.
-Ability to communicate effectively, both orally and in writing.
-Valid CPR Certification or the ability to complete CPR Certification within 1-2 months of hire date.
Preferred
-One-year experience in Targeted Case Management services.
