Job Description

Band 4 - £26,530 to £29,114

Full-time post - 37.5 hours per week (part-time available upon discussion with hiring manager)

An opportunity has arisen for a band 4 Associate Practitioner to join us. The Community Stroke Rehabilitation Team is a nationally respected Early Supported Discharge Team.  It is an essential part of the Portsmouth Hospitals Stroke Pathway, ensuring the timely transfer of patients out of hospital for rehabilitation at home. 

The post holder will be working with a large but very closely working MDT with competencies across several disciplines: occupational therapy, physiotherapy, speech and language therapy and nursing. Your willingness and ability to learn new skills as working across MDT is essential, as well as excellent communication skills.

You will be working mostly Monday to Friday 7.30/8am -15.30/16.00 plus providing some weekend cover on a rota basis.  You will be carrying out but also progressing patient rehabilitation under indirect guidance. Experience of working with patients with neurological deficit and community working would be desirable. You need to be a confident car driver and user, as the team covers a large geographical area.

Qualifications

Essential

  • Level 2 Functional skills (English & Mathematics GCSE grade 4)
  • Level 3 Qualifications:  NVQ 3, Diploma, T level or relevant experience or relevant short courses

Desirable

  • Health and Social Care Foundation degree or equivalent

Experience

Essential

  • Significant experience of working in a relevant health or social care setting

Desirable

  • Knowledge and experience of working with stroke patients.
  • Experience of working in community setting

Skills & Knowledge

Essential

  • Full driving license with appropriate car insurance
  • Ability to work as part of a team
  • Recognise own strengths and limitations

Desirable

  • Competent IT skills
  • Knowledge of relevant multi-disciplinary team members roles (nursing/physiotherapy/occupational therapy/speech and language therapy)
