Job Description

An exciting opportunity has arisen for a Band 6 Sister/Charge Nurse who is experienced and passionate to join the Community Stroke Rehabilitation Team (CSRT).

CSRT is an integral part of the Portsmouth Hospitals Stroke pathway, providing an ESD and CRT combined service across Portsmouth, Fareham and Gosport and South East Hampshire. We continue to build on our reputation for innovation and excellence, consistently delivering outstanding performance, whilst developing the skills and abilities of our staff.

Key Responsibilities

Your main role will be providing clinical leadership to the other nurses and HCSW within the team. You will manage sickness, provide supervision and support with learning and development. You will also carry out nursing assessments and write activity plans to support patient needs and rehabilitation. This will include advice and education on secondary prevention, including diabetes. alcohol, smoking, medication and much more, as well as support for carers and families. The patient caseload is varied and challenging to support patients navigating life after stroke. This post is part of a large interdisciplinary team, as well as needing to liaise with other health professionals such as GPs, social services and mental health teams.

You must be competent and a confident driver as CSRT cover a large geographical area. CSRT covers seven days a week from 8am to 9pm. A mix of working patterns is available, including long days.

Qualifications

Essential

  • Current RN +/-, registration with NMC and any other relevant post registration qualifications
  • BSc Degree in relevant field
  • Post Reg. qualifications for specialty area
  • Leadership development programme
  • Teaching qualification/experience and accredited NMC before mentorship course

Desirable

  • Masters degree or evidence of working towards

  • Significant demonstrable experience within specialty with evidence of professional development
