It is a full-time position.

Job Description

The Care Coordinator (CC) is a critical member of the Chronic Care Management team. He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients enrolled in the Chronic Care Management Program. Responsible to the clinical team and reports to the Clinical Manager.

  • Provide administrative support for the Chronic Care Management Program
  • Assist with the implementation or finalization of patient orders that do not require an RN to complete
  • Facilitate medical records management for outside documents that are critical for patient management, i.e. laboratory results, discharge summaries, medical documentation from specialists and primary care provider
  • Assist with order and management of medical supplies for the clinical team
  • Assist with oversight of vaccine storage and handling
  • Assist with order and management of office supplies for the clinical and office based colleagues
  • Support scheduling appointments and reminder calls or texts for Chronic Care Management appointments
  • Assist with referrals and referral management
  • Assist with follow-up to assure needed DME is ordered and delivered
  • Assist with making community provider appointments and appointment reminders as needed
  • Support a variety of data entry functions
  • Complete other duties that support the Chronic Care Management program as assigned

Qualifications

  • Minimum of a High School diploma
  • Bilingual preferred in Spanish
  • Understanding of medical terminology
  • Completion of a Medical Terminology course preferred
  • Medical office experience preferred
  • Strong work history
  • Working knowledge of computers and ability to manage data within an electronic system
