Job Description
Position Summary:
Responsible for providing medical services and support to clients through group facilitation, individual client sessions, consultation, staffing, care coordination, and medication observation.
Essential Functions:
- Facilitate gender-specific experiential, and education groups and lectures within scope of practice
- Conduct individual client sessions as requested
- Provide health consultation services to clients and clinical staff within scope of practice
- Participate in weekly multidisciplinary staffing meetings
- Complete accurate, detailed, and timely documentation in clients’ electronic medical record
- Provide care coordination of services between clients and on-site medical clinic
- Provide support to medication observation team, ensuring clients receive and take medications as prescribed
- Maintain a professional relationship with clients while following a strict code of ethics
Qualifications
Knowledge/Skills/Abilities:
- Must demonstrate competence in working with clients with co-occurring disorders; through education, training, and experience
- Ability to work in alignment with company values, goals, policies and procedures
- Knowledge of MN Statute Chapter 245G and licensing and its application to the department and company’s policy and procedures; or ability to obtain knowledge after hire
- Ability to establish and maintain effective and cooperative relationships with co-workers
- Ability to maintain excellent interpersonal, verbal, and written communication skills in English
- Ability to establish priorities, manage projects, and meet deadlines
- Ability to work both independently and as part of a team
- Proficiency in Microsoft Office programs and electronic medical record software program; or ability to obtain proficiency after training
Education/Licensure/Certification:
- Must be licensed in good-standing as an RN in the state of Minnesota
- Must obtain and maintain required continuing education units and training
Additional Requirements:
- Must be 18 years of age or older
- Must be free of chemical use problems for at least the two years immediately preceding hire
- Must pass a background check through the Minnesota Department of Human Services
