Park Avenue Center provides research-based gender-specific drug and alcohol treatment in south Minneapolis, Minnesota. First established in 1979, we are a community-based program that proudly and ethically serves Minnesota’s less fortunate individuals in gorgeous settings. Providing beautiful environments shows our passion and respect for clients and their recovery. Using evidence-based best practices, we have maintained a reputation for the highest quality of services and care. Clients we serve typically struggle with multiple barriers to recovery, including mental illness, homelessness, financial problems, legal issues, and an overall lack of emotional support. We provide stunning, free, supervised off-site housing for clients in need. Programs are issue oriented including topics such as thought process, mental illness, trauma, anger management, relapse prevention, spirituality, relationships, self-esteem and self-image, codependency, budgeting, nutrition, education, physical movement, parenting, and 12-step meetings. JOB SUMMARY

Under the supervision of the Social Work Supervisor, the Social Worker works with clients in obtaining appropriate, necessary resources. The Social Worker coordinates and collaborates with clients, clinical staff, community-based resources and others to help obtain stable, supportive housing and additional resources for clients upon transitioning out of treatment.

II.        DUTIES AND RESPONSIBILITIES

1. Develops and maintains working knowledge of community resources available to clients, including: health insurance options, housing, medication resources and support groups.
2. Participates in client staffing to maintain consistent communication with counseling staff.
3. Meets individually with clients determining needs and facilitating resources; helping clients in their transition from intensive outpatient programming.
4. Facilitates a variety of groups including, but not limited to, activity and psychoeducational.
5. Meets with families, probation, and social workers to develop transition and contingency plans.
6. Maintains appropriate charting to communicate clients’ transitional plans.
7. Coordinates correspondence with Probation, CPS, and other concerned persons.
8. Performs other duties as assigned.

1. Must be LSW or MSW, currently licensed in the State of Minnesota.
2. 2,000 supervised hours working with individuals with substance use disorders preferred.
3. Computer literacy is required; proficiency is desirable but not required.
4. Prior experience with an Electronic Health Records system strongly preferred.
5. Must be able to maintain positive public relations with referring agencies and the business community at large.
6. Must possess good verbal skills and present a positive professional image.
7. Must have a working knowledge of issues regarding confidentiality.
8. At the time of hiring, must be free of chemical use problems for at least the two years immediately preceding and sign a statement attesting to that fact.
9. Must pass background check through the Department of Human Services.

This is a full-time, benefits-eligible position working Monday - Friday days.
