Job Description

FAMILY MEDICINE-PRIMARY CARE NURSE PRACTITIONER – BOSTON AREA


We are looking for an Experienced Family Medicine/Primary Care Nurse Practitioner to join an exciting primary care practice providing medical, pharmacy, dental, behavioral health, and vision services to diverse underserved communities in Allston, Brighton, Waltham and surrounding areas


The practitioner will manage a panel of low income, culturally and linguistically diverse patients from newborns through seniors using an integrated, team-based approach to care by Evaluating and treating patients, writing prescriptions, providing preventive health care advice and case management, assessing psychological impact of illness and treating or appropriately referring patients, for additional evaluation and treatment


The state of the art facility is affiliated with 3 Harvard Medical School teaching hospitals, uses an Electronic Health Record, and is working toward achieving Meaningful Use.


Qualifications:

MA License as Advanced Practice Nurse Practitioner

Board Certification

2-3 years prior ambulatory experience required

Experience working in a diverse culture preferred

Fluency in medical Spanish and/or Portuguese preferred
