Job Description
The Director of Clinical Services leads our clinical team in providing high-quality care in a home health environment. This leadership role is crucial for ensuring compliance, overseeing medical records, and enhancing patient monitoring processes. The ideal candidate will possess a strong background in case management, supervising staff, and developing care plans that meet the needs of Home Health Patients.
Responsibilities
- Lead and manage the clinical team to ensure the highest standards of care are maintained.
- Oversee patient monitoring activities to ensure safety and well-being.
- Develop, implement, and evaluate individualized care plans for patients.
- Ensure compliance with all regulatory requirements related to Home Health.
- Supervise staff, providing guidance and support as needed.
- Conduct regular assessments of clinical practices and outcomes to improve service delivery.
- Collaborate with other departments to enhance overall patient care experience.
- Provide training and mentorship to clinical staff to promote professional development.
Qualifications
Qualifications
- Proven experience in a leadership role within clinical services, preferably in a home health settings.
- Strong knowledge of Medicare rules and regulations as it relates to Home Health, medical records management, patient monitoring, and case management practices.
- Exceptional supervisory and employee relations skills.
- Excellent communication and interpersonal skills to foster a collaborative work environment.
- Ability to develop comprehensive care plans tailored to individual patients clinical needs.
- Relevant certifications or licenses in nursing or healthcare administration are preferred.
