Job Description
The Clinical Nurse, Enrollment Hospital at Home AMC reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities:
Responsible for accurate and efficient patient identification for the NM Hospital at Home program. Identifies eligible patients and conducts clinical and social eligibility and safety screening while collaborating with the Admitting provider and Clinical Coordinator. This role also communicates with the ED/Inpatient care team, the patient and family and the ED/Inpatient Case Managers at times to facilitate transfers to NM Hospital at Home. This role is in many instances the patients’ first introduction to NM Hospital at Home and because of this, the role requires a strong understanding of the program and the ability to present the program accurately, addressing any questions raised by the patient, family or care team.
The nurse is present in the ED/Inpatient setting, proactively, case-finding by engaging in rounds and huddles, as well as independently identifying patients throughout the shift to maintain goals of Hospital at Home growth and census.
- Gathers appropriate clinical information to identify patients based on geographic region and plan of care
- Interacts with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification with the goal of successful transfer to NM Hospital at Home
- Interacts with the Inpatient interdisciplinary team and Inpatient Care Facilitators to assist patient identification with the goal of successful transfer to NM Hospital at Home
- Collaborates closely with the NM Hospital at Home Admitting Provider when prospective patients meet eligibility criteria
- Obtains consent from patient to transfer to NM Hospital at Home
- Communicates and collaborates with NM Hospital at Home Clinical Coordinator regarding patient transfers
- Works with care areas to manage patient flow and placement to support Home Hospital transfers and census goals
- Utilizes analytical and critical thinking in decision making and judgment that is aligned with organizational objectives
- Exhibits excellent and effective communication skills that promotes patient centered care, relationship building, and professionalism
Qualifications
Required:
- Graduate of an NLN accredited school/college of nursing
- Baccalaureate degree in Nursing or higher
- Current State of Illinois, Registered Professional Nurse license
- Basic Life Support (BLS) CPR through American Heart Association.
- Analytic ability (conceptual and quantitative)
- Oral, listening and written communication skills
- Strong human relation skills to interact effectively and collaborate with others in difficult situations
- Use of a keyboard to enter or retrieve data
- Minimum 1 year emergency department, home hospital, or inpatient experience
- Travel required within designated geographic area
- At point of hire and annually, required to have a valid driver’s license and an annual driving abstract
Preferred:
- Minimum 3 years emergency department, home hospital, or inpatient experience
