The Pharmacy Clinical Coordinator reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

Under minimal supervision, acts as the departmental resource person to develop, implement and coordinate the clinical/educational services of the Pharmacy Services Department and to disseminate pharmaceutical information to the medical staff and hospital departments.

Clinical pharmacy programs

Develops, implements and coordinates the departmental pharmacotherapeutic dosing services with 24 hour consultation (such as but not limited to: pharma cokinetic,hyperalimentation, and anticoagulation).

Develops, implements and coordinates additional clinical pharmacy services as needed to meet the needs of the patient, the hospital and the medical staff.

Offers suggestions for new clinical programs the Pharmacy Services Department should pursue, due to issues arising within the department/hospital or from recommendations from American Society of Health System Pharmacists, The Joint Commission, or other organizations.

Keeps abreast of current clinical pharmacy concepts, ideas and programs.

Maintains a strong medical knowledge data base, including the latest advances in drug therapy.

Drug information and education

Coordinates requests for drug information, providing verbal and/or written documentation to the requestor as appropriate.

Prepares/edits the "Pharmacy Newsletter", quarterly department PharmacyLink and other related departmental publications.

Develops, implements and coordinates pharmaceutical education programs for the patients, medical staff, nursing staff, pharmacy staff, other hospital departments and community.

Researches and prepares special reports and projects as requested by pharmacy/hospital administration.

Maintains/updates the departmental drug information files and reference library.

Makes recommendations for additions, changes and updates to reference materials.

Coordinates College of Pharmacy externship rotation for the medicine rotation. Includes evaluating and grading student activities.

Coordinates the hospital's American Society of Hospital Pharmacists PGYI Residency Program.

Continuous quality improvement of drug therapy

Makes recommendations to optimize drug utilization practices. Disseminates recommendations to appropriate hospital and medical staff committees and/or individual practitioners.

Develops, implements and monitors hospital/departmental protocols involving the use of pharmaceuticals.

Develops, implements and coordinates the drug utilization review activities of the department. Selects drugs for review in consultation with pharmacy/hospital administration, medical staff and other hospital departments.

Compiles, prepares and reports on drug utilization review activities to appropriate hospital and medical staff committees, including hospital-wide Performance Improvement Council and Safety Committee.

Administration and personnel

Interview, hires, evaluates, counsels and disciplines Clinical Pharmacists. Provides Director of Pharmacy with input regarding clinical evaluation of Staff Pharmacists.

Conducts the orientation and training for all new Clinical Pharmacists.

Actively participates in and contributes to all assigned hospital committees.

Develops, implements, and coordinates the documentation of clinical pharmacy activities.

Compiles and reports all departmental clinical activities on a regular basis to pharmacy administration.

Actively participates in the departmental quality improvement program.

Miscellaneous

Practices safe work habits in compliance with hospital's exposure control plan.

Functions as a Clinical Pharmacist and/or Staff Pharmacist as needed.

Performs other inherent duties, related, but not limited to the above.

Graduate from an accredited school of Pharmacy. Must be registered or eligible for registration as Pharmacist in the State of Illinois. Advanced degree (Pharm. D. or M.S.) in Clinical Pharmacy and has completed an American Society of Health-Systems Pharmacists approved PGYl and or PGY2 residency. American College of Clinical Pharmacy board certification preferred. Minimum of three years hospital experience as a clinical pharmacist.
