Job Description

The Coding Specialist HCS-D reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The RHIT  will be able to code all types of Home Health and Hospice charts. Required to maintain accuracy and productivity, be detail orientated, have the abilitiy to work independently, as well as adapt to change. Demonstrates ability to understand the clinical content of a health record including the most complicated records. Must also be able to communicate with physicians in order to clarify diagnosis/procedures and sequencing of diagnosis. Ability to demonstrate knowledge of and utilize auditing skills related to coding quality and compliance.

  1. Codes and sequences diagnosis/procedures according to Home care and hospice ICD coding official guidelines                      
  2. Conducts chart reviews and review documentation performance. This is accomplished by reviewing OASIS and reviewing clinical documentation, performing side-by-side or electronic, telephonic  education
  3. Evaluate documentation to ensure that diagnosis coding is supported and meets specificity requirement to support clinical indicators, and quality measures
  4. Query providers regarding missing, unclear or conflicting health record documentation by requesting and obtaining additional documentation within the health record
  5. Assist  as needed with team’s coding goals to meet departmental goals/deadlines
  6. Develop relationships with clinicians and staff, communicate coding and documentation guidelines and requirements of the Risk Adjustment program to ensure correct coding and documentation
  7. Analyze data to prioritize clinician educational reviews. Implement education, where necessary, and provide formal training to clinicians, auditors and staff regarding coding and documentation
  8. Assit as needed with team’s coding goals to meet departmental goals and guidelines
  9. Responsible for managing  requests for release of information, assuring proper verification of the source of the request, as well as locating, copying and sending the record to either the requestor, or in the case of a subpoena, to Palos Community Hospital
  10. Works collaboratively with the care team, home care and hospice care team to ensure the plan of care has a unique perspective about the patient status, goals, barriers/facilitator to care and the care plan interventions that they believe will best help the patient achieve the desired health and well-being outcomes
  11. Adheres to PCH Home Health policies regarding all aspects of HIPPA, Privacy and confidentiality Rules, dress code and attendance.
  12. Other duties as assigned.

 

 

    Qualifications

    Required:

    • Certification as a Registered Health Information Technician (RHIT), certficiation as a Certified Coding Specialist (CCS) or Registered Health Information Administrator (RHIA) or HCS-D, Home Care Coding Specialist-Diagnosis
    • High School/GED

    Preferred:

    • 3 years experience  with Health Information Management, and home care or hospice experience.
    • Associate Degree

    This position is Remote. Work remote from Illinois, Indiana, Iowa, and Wisconsin
