Position Title: Primary Care Provider – FNP/PA

• Became a Federally Qualified Health Center in 2003 and now offers primary medical, dental and behavioral health services for the entire family through nine sites including six Fremont-based primary care sites, a dental site and mobile clinic.

• Provides a wide spectrum of health education, healthcare, and social services to everyone without regard to financial position, ethnicity, language, culture, sexual orientation or any other criteria.

Your Role with the Company:

• Full-Time, 40hrs/week, Flexible Schedule

• Clinic is open 8a-7p M-Th, 8a-5p F-Sat   

• The Family Nurse Practitioner/Physician Assistant will be responsible for Primary Care services, supporting individual lifestyle changes, and helping individuals manage their chronic conditions.

• This FNP/PA will make a big impact within the community, seeing all ages, an average of 16-20 patients/day

• You will have the opportunity to build strong relationships within the community and develop continuity of care with your patients
