Job Description
The Outreach Nurse supports the efficient delivery of community-based vaccination and STI services through outreach events. This role works closely our Community Health and Wellness partners and with the Infection Control Nurse and exercises independent judgment in monitoring outreach activities and associated clinical functions. The Outreach Nurse actively participates in quality improvement initiatives, program monitoring, and ongoing education to stay current with advancements in vaccines and STls.
Additionally, the Outreach Nurse ensures patient safety during outreach events and plays a key role in expanding NATIVE HEALTH's mobile, school-based, and community-based vaccination and STI services. This work primarily serves underserved and minority populations throughout Maricopa County.
This position is dedicated to outreach-based vaccination and STI services under the Shield for Life program and reports directly to the Health Services Director.

KEY ACCOUNTABILITIES:
- Monitors all outreach events and their clinical operations and related functions. Measures of Success:
- Maintains current knowledge of vaccine updates and STI testing and treatment guidelines.
- Determines appropriate vaccine vials, STI testing supplies, and related materials required for each outreach event.
- Ensures timely and accurate documentation of vaccine distribution, retrieval, testing supplies, and required logs.
- Prepares or oversees the preparation and administration of vaccines at each event in accordance with clinical policies, standing orders, and scope of practice.
- Minimizes vaccine wastage through effective planning and consistent communication with the registration team regarding anticipated attendance.
- Conducts STI testing in outreach settings and coordinates timely follow-up for individuals with positive results, including notification, education, and linkage to care.
- Reviews vaccine screening questionnaires with patients to verify eligibility prior to administration.
- Ensures emergency medical equipment is present at all events and routinely inspected for readiness.
- Provides timely and appropriate responses to patient inquiries and concerns.
- Collaborates with outreach staff to ensure smooth and efficient event operations, including scheduling, pre-registration, clinical readiness, logistics, mobile unit preparation, deployment, breakdown, and post-event follow-up.
- Directs patient and community inquiries regarding vaccines, STI testing, and treatment to appropriate internal staff.
- Monitors vaccine recipients during observation periods and promptly provides assessment and intervention as needed to ensure patient safety.
- Reports any vaccine wastage in accordance with Vaccines for Children (VFC) program guidelines.
- Participates in culturally responsive and trauma-informed outreach efforts that
promote health equity and improve access to preventive services for underserved and minority populations.
- Responsible for the immunizations program and outreach-based STI testing program related to outreach events
Measures of Success:
- Coordinates immunization and outreach-based STI testing services to support efficient event clinic operations.
- Supports baseline development, performance monitoring, KPI tracking, grant reporting, and quality improvement initiatives aligned with program goals, including outreach volume, immunization uptake, STI screening rates, prevention education, and linkage-to-care outcomes.
- Assists in tracking program outputs and outcomes, including mobile vaccination events, school-based clinics, culturally tailored outreach efforts, number of individuals reached, vaccines administered, individuals screened or tested, and successful linkage to care following positive STI results.
- Orders vaccines, testing materials, and outreach supplies in a timely manner and maintains effective inventory control.
- Regularly updates inventory logs to monitor supply levels and proactively reorder as needed.
- Maintains accountability for temperature monitoring devices (data loggers) and ensures readings are within acceptable range prior to placing vaccines in refrigerated or frozen storage.
- Accurately documents vaccine administration in the electronic health record (EHR) and the Arizona State Immunization Information System (ASIIS) and completes all required reporting and data entry.
- Reports any vaccine adverse events in accordance with the Vaccine Adverse Event Reporting System (VAERS) requirements
- Provides nursing services to patients following the Arizona State Board of Nursing scope of practice.
Measures of Success:
- Patients' health status is assessed utilizing effective nursing protocols, screening, and observation.
- Informs the Medical Practitioner of changes in the patient's condition, medication, and diagnostic results.
- Collects information, data, and documents in a timely and accurate manner in the patient's electronic medical record.
- Addresses patient concerns, inquiries, and feedback, including returning patients phone messages, if any, timely.
- Provides patient education related to immunization, STI prevention, testing, treatment follow up and referral services in culturally responsive & community-centered manner.
- Support timely referral or warm hand-off to primary care, behavioral health, or appropriate services as needed.
- Supports the organization as needed. Measures of Success:
- Assists with outreach program needs and related community-based clinical activities, as required.
- Participates in NATIVE HEALTH requirement of eight (8) hours of non-position related outreach.
- Attends and actively participates in required in-services, conferences, training sessions and division staff meetings.
- Perform other duties as assigned by the supervisor.
Qualifications
Qualifications / Experience / Requirements
- Graduation from an accredited school or college of nursing is required.
- Current licensure to practice nursing in the State of Arizona as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) is required.
- Minimum of three (3) years of nursing experience is required.
- Ability to work a flexible schedule, including weekends and evening hours, as needed.
- Ability to travel throughout Maricopa County to various community-based locations.
- Ability to stand for extended periods and lift and carry supplies as required.
- Ability to work outdoors and in non-traditional healthcare settings.
- Demonstrates dependability, punctuality, reliability, and the ability to work effectively with others while meeting deadlines.
- Maintains a professional demeanor in behavior, appearance, and all organizational materials produced.
- Possesses strong critical thinking, problem-solving, and customer service skills with a high level of professionalism.
- Proficient in computer use, including Microsoft Office applications (Word, Excel, Access, PowerPoint) and internet navigation.
- Must possess a valid and unrestricted Arizona driver's license (or obtain one within 30 days of hire if relocating from out of state). Must have no DUI/DWI or reckless driving convictions within the past five (5) years and no more than two at-fault
accidents within the past three (3) years. Must remain insurable under NATIVE HEALTH's liability auto insurance policy.
- Must successfully pass a criminal background check and obtain a Class I Fingerprint Clearance Card within ninety (90) days of employment.
- Must pass pre-employment drug and alcohol screening and comply with random testing during employment.
- Must provide up-to-date immunization records.
- Must provide documentation of a current tuberculosis (TB) screening (within the past 12 months) upon hire and maintain annual compliance.
- Must obtain CPR certification within ninety (90) days of employment and maintain a valid certification throughout employment.
Preferred Qualifications
- Experience with immunization programs, including vaccine storage and handling, and reporting to state immunization registries.
- Experience supporting STI, HIV, HCV, or other infectious disease screening, including linkage-to-care coordination.
- Experience providing nursing services in community-based, mobile, school-based, or outreach settings.
- Bilingual in English and Spanish preferred; proficiency in additional languages relevant to the service population is highly valued.
Knowledge, Skills, and Abilities
- Knowledge of immunization programs, vaccine storage and handling, STI screening workflows, and state reporting requirements is preferred.
- Demonstrated commitment to improving the health status of minority, underserved, uninsured, and vulnerable populations, including a willingness to serve underinsured communities.
- Proficiency with computer applications, including Microsoft Office, Adobe, registries/medical databases, and electronic health record systems (EMR.
